That’s the part the X-rays don’t lie about. Severe degenerative disc disease, L4-L5, with osteophytes pressing directly on the nerve roots. The radiologist used the word agony. Twice. The patient — a fifty-eight-year-old woman who had hauled dried fish for twenty years — was walking around a fishing village on a remote Filipino island as if pain were a concept that applied to other people.

Dr. Marco Reyes, a first-year resident at Philippine General Hospital in Manila, is not interested in miracles. He is interested in why his mother feels nothing. He came to the village expecting a faith healer’s con. He found something worse.

The houses are given to outsiders for free. Strangers know his name before he gives it. Three hundred and fifty villagers walk up to a cave every night to drink from a pool at the roots of a single pale tree. A man who threatened to burn that tree is found dead on a busy trail in broad daylight, and every villager has an alibi clean enough to be a courtroom exhibit. His sister — who has never lied to him in her life — is suddenly lying. And every time Marco thinks about going back, something in his head feels warm.

Working from a partial sample that survived a sister’s quiet sabotage, Marco and a mycologist friend in Manila piece together what is really growing in the village water. The pattern of pain that vanishes. The synchronized alibis. The organism that medical science has not yet learned to name. And the recordings of his own thoughts, increasingly, that no longer feel entirely his own.

The deeper Marco digs, the more he understands that the village does not need to convert him. It already has.

The most dangerous thing on that island was never the people who smiled.
